Tracheobronchial Injuries

aGöğüs Cerrahi ABD, Fırat Üniversitesi Tıp Fakültesi, ELAZIĞ Tracheobronchial injuries are rare but potentially life-threatening. Airway compromise can lead death acutely. Accompanied injuries especially vascular ones account for the high mortality. Definitive airway management and early diagnosis are the keys to ensuring acceptable clinical outcomes. The enhanced diagnostic capability of computed tomography has changed the evaluation of these patients. However, CT scan findings only have suggestive capacity. Bronchoscopy can give definitive diagnosis as well as site, extent and nature of airway injury. Given the devastating nature of these injuries, especially if the diagnosis is delayed, additional evaluation (contrast studies, endoscopy or angiography) should be used liberally if there is any concern for tracheobronchial, esophageal, or vascular injuries. A large continuous air leak rises from the tube thoracostomy in patients with tracheobronchial injury. In severe injuries, high-frequency or “jet” ventilation may be lifesaving. Primary repair is essential for management, even in delayed cases. Buttressing repair can fasten healing.
