Injury Of Diaphragm

aGöğüs Cerrahisi ABD, Dicle Üniversitesi Tıp Fakültesi, DİYARBAKIR Diaphragm ruptures may established due to both penetrant and blunt traumas of thorax and abdomen. Ratio of rupture varies between 1% and 5%. Injuries of diaphragm often accompany multiple organ injuries. Diaphragm ruptures that developed relating to suddenly raise intraabdominal pressure are left side localisation of blunt traumas; and ratio of left/right is 5/1. Liver protects diaphragm partially, in the right side. Causes of penetrant diaphragm injuries are sharp, penetrative tools and firearms.
The semptoms of diaphragm rupture are breathing stress, cardiac findings, trachea deviation, hearing of gut voices in the thorax. It must be consider diaphragm rupture in the trauma patients, primarily. Directly chest graphy, ultrasonography, computer tomography, fluoroscopy, endoscopy and thoracoscopy by video are usefull in the diagnosis.
After diagnosis, rupture must be repair by surgically before developing complications as incarceration, strangulation. Laparotomy is preferred due to intraabdominal organ injury, especially for left side, in the acute phase. Thoracotomy may performed in the right side, if is not considered intraabdominal injury.
